Should I See a Chiropractor for Sciatica?

If you’ve been Googling “sciatica” at 1am because the pain shooting down your leg is keeping you awake, you’re not alone. Sciatica is one of the most common complaints we see at Zest Chiropractic — and the good news is that for most people, it responds very well to chiropractic care.

But before we talk about treatment, it helps to understand what sciatica actually is — because there are a lot of misconceptions out there.


What Is Sciatica?

First, an important distinction: sciatica isn’t a diagnosis. It’s a symptom.

Sciatica refers to pain in the leg that originates from irritation or compression of the lumbar nerve roots that make up the sciatic nerve. That’s why the pain can travel from your lower back, through your buttock, and down into your leg or foot — sometimes as a sharp, burning, or electric sensation; sometimes as a dull ache, numbness, or tingling.

The underlying cause — the thing that’s actually irritating or compressing the nerve — is what needs to be identified and treated. In our experience, the vast majority of sciatica cases are mechanical in origin. That means the spine has been placed under sustained or repetitive loading that has caused structural changes over time.

Common culprits include:

  • Prolonged sitting — particularly with poor posture, which shifts the load through the lumbar spine in ways it isn’t designed to handle
  • Physical occupational demands — heavy lifting, repetitive bending or twisting
  • Disc herniation — where the soft material inside a spinal disc pushes outward and presses on a nearby nerve root

The Truth About Disc Herniations

One of the most common things patients say to us when their sciatica comes and goes is: “I think my disc keeps popping out and going back in again.”

It doesn’t actually work that way. Spinal discs don’t mechanically pop or slide out of position, then go back in again. What happens is that small tears can form between the layers of cartilage that make up the disc. Then, the soft inner gel of the disc pushes through the tears forming a bulge.  Although you can’t push the disc material back in, the symptoms often resolve as your body naturally absorbs the protruding material. Inflammation around the nerve roots neat the disc can also settle. That’s why sciatica symptoms from a disc herniation can fluctuate.

Understanding this matters for treatment. Discs are commonly injured through twisting movements, which is why at Zest Chiropractic we use Advanced Biostructural Correction (ABCTM) — a method that does not use adjustments that involve twisting motion. This makes it a particularly safe and appropriate approach for people with sciatica due to a disc herniation, because it avoids the risk of aggravating an already compromised disc.


Is Chiropractic Right for My Sciatica?

Chiropractic care is well suited to sciatica that is:

  • Recent onset — you’ve had it for days or weeks and want to address it before it becomes chronic
  • Chronic but manageable — you’ve had recurring episodes and want to get to the root cause rather than just managing flare-ups
  • Mechanical in origin — related to posture, loading, or disc changes rather than a serious underlying condition

When to Seek Urgent Medical Attention

There are a small number of cases where chiropractic is not the appropriate first step and it’s important to be clear about this. Red flag symptoms that require immediate (same day) medical investigation include:

  • Loss of bladder or bowel control
  • Numbness in the groin or genital area
  • Bilateral symptoms – pain, numbness, or weakness affecting both legs
  • New onset of sciatica in someone with a history of cancer
  • Unexplained weight loss

If you have any of these symptoms, please seek urgent medical care rather than chiropractic assessment. In the case of bowel, or bladder dysfunction and groin / genital numbness go immediately to A&E. These symptoms can indicate serious spinal pathology that requires a different approach.

For everyone else? You don’t need to suffer, 80% of sciatica cases respond well to conservative care. In most cases surgery is not required.
